The following is translated from Rabbi Eliezer Berland’s compendium of prayers in the Refuah Shleimah booklet. To get the prayer in the original Hebrew, you can buy the booklet HERE.
Master of the world, please send a complete recovery to my eyes, in the merit of, “this (17) month (317) will be for you (90) the head of the months” = 424 the numerical value of “and I will find (138) favor (58) in the eyes (142) of Hashem (86)” = 424.
And in the merit that from today and onwards that I merit to fulfil the saying in the lessons of our holy teacher to close my eyes forcefully, and to close my eyes from seeing this world.
And may I merit to make complete teshuva from this day and onwards on all the blemishes of my eyes, that which I have sinned and transgressed and negligently sinned before You from my beginning until today, whether in this incarnation or in other incarnations.
And all the damaging destructive forces that I have created on account of not guarding my eyes will be destroyed and disappear in the blink of an eye, and they will no longer be able to damage me.
And may I make complete teshuva with a broken and dejected heart, and from today may I merit to have tremendous happiness which will enter into all of my 248 limbs and 365 sinews.
And may I merit from today onwards to guard my eyes with the utmost guarding, to forcefully close them and to close them completely, to press them with my ten fingers, and never to open them again ever from now and forever.
And in the merit of this heal them in the blink of an eye, Amen.
And I will no longer look at all on the desires of this world, which come from the leprosy of the snake, and may I merit to remove the skin of the snake which is upon me, and to receive a holy and pure body from Gan Eden, from now and forever.
